CounselorPosition SummaryThe Counselor is responsible for gathering data, documenting problems, and establishing a course of treatment. Provides direct addiction treatment services to adult patients and family members.Essential Duties and Responsibilities include the following and other duties as assigned : Provide individual and group counseling to patients assigned to caseload.Direct training groups and seminars. Provide therapeutic leadership in group counseling sessions.Function as a liaison with other community-based agencies and law enforcement agencies to ensure continuity of treatment (referrals) and compliance with legal obligations.Complete individual chemical dependency assessment of new patients, ensuring accuracy and completion of the assessment process.Develop individual treatment plan (ITP) for new patients within established time frames, verified by the patient’s signature.Update new patient charts within established time frames, ensuring treatment plans reflect current problems / goals.Work with medical staff in securing urine samples for monthly urinalysis, documenting results in the patient’s chart and program software.Actively involve in Diversion Control through execution of random bottle recalls.Provide necessary forms to the Medical Director for special requests regarding medications.Ensure patient confidentiality by properly completing release of information to outside referrals and contacts.Maintain compliance of assigned patient files with DHHS regulations and the Policy and Procedure Manual of Rowan Treatment Associates, Inc.Complete patient discharge forms within seven (7) days from the date of discharge and submit to the Program Director.Contribute to ongoing assessment and treatment planning through participation in daily reports, case reviews, and treatment teams.Provide patient education relating to chemical dependency and the recovery process.Provide supportive counseling or crisis intervention services as necessary.Educate the public and professional community regarding opioid treatment programs.Participate in a program of professional development, working towards and maintaining professional credentials.Participate in Continuing Quality Improvement and report on assigned CQI monitors.Job Requirements Job Standards / Minimum Qualifications : Four-year degree in counseling or related field, or experience in a similar field. Candidate must possess an active license to practice in the state of NM. Acceptable licenses include LSAA, LADAC, LMHC, LPCC, LMFT, LMSW, or LISW. Must have a valid driver’s license or state-issued ID.Physical Requirements : Frequent sitting at a desk or computer while typing, completing paperwork, or meeting with patients. Occasional lifting / carrying of paperwork, files, or office supplies up to 20 lbs.Occasional push / pull exerting up to 5-10 lbs. of force, opening doors, and file drawers.Occasional kneeling / crouching and / or ability to reach lower file drawers.Mental Requirements : Position requires constant mental alertness, attention to detail, and a high degree of accuracy in completing all assignments.
