What are the responsibilities and job description for the LPN position at New Northbridge Elementary School?
The Licensed Practical Nurse (LPN) will work under the supervision of the Registered Nurse (RN) to support the overall functioning of the school health office, providing direct care and health services to students as needed. In addition, the LPN may be assigned to deliver individualized, 1:1 medical care for a student with complex health needs, following the student's Individualized Health Care Plan (IHCP), Individualized Education Program (IEP), and/or Section 504 Plan. The role requires flexibility, strong clinical skills, and the ability to collaborate with school staff, families, and medical providers to ensure student safety and well-being throughout the school day.
Responsibilities:
1. Provide student direct care and implementation of an individualized health care plan
2. Experience monitoring juvenile diabetics, pediatric tracheostomies, and GI tube feedings
3. Ability to interact with all students in a caring and compassionate manner
4. Accompany student to General Education classroom and provide as outlined in the
Individualized Health Care Plan (IHCP), Individualized Education Program and/or
Section 504 Plan.
5. Collaborate with program personnel to create the best learning environment regardless
of individual’s health care needs
6. Provide medically prescribed interventions including medication administration
(based on state regulations) and nursing procedures and provides nursing care to the
chronically ill children
7. Assist individual with daily living activities, e.g.: lunch, bus load, personal care,
toileting
8. Maintain comprehensive school health records, including maintaining an electronic
log of the day’s medical treatments
9. Provide coverage in the school’s health clinic when required
10. Participate in required professional development
11. Perform other duties as assigned by the supervising RN/School Nurse Leader,
Building Principals, and/or Director of Pupil Personnel Services
