The Case Manager for Adult Mental Health Services, provides case management and clinical services for individuals with mental illness, substance disorders use and/or developmental disabilities and their families. Services include supportive counseling, individual and family support, crisis intervention, assessment, Individualized Service Plan (ISP) development, psycho-education, and referrals to clients and their families. Develops and maintains working relationships with: individuals served, families, additional service providers, and other community agencies as needed to ensure quality services. Works as part of a treatment team to ensure services and resources are coordinated among providers as needed to help individuals served live and function optimally in the community.
Case Manager I: $16.73
Case Manager II: $17.56
Case Manager III: $18.44

Education/Experience Required:
- High School Diploma with at least one year of experience in a setting working with individuals with mental illness or substance use disorders is required.
- Bachelor's Degree is preferred in psychology, counseling, social work or special education, but will also consider an unrelated degree.
- Experience in a setting working with individuals with mental illness, substance use disorders and/or developmental disabilities is preferred
Salary : $18 - $18
