Duties

- Utilize HEDIS measures to evaluate the quality of care provided and identify areas for improvement.
- Maintain accurate medical records and documentation in compliance with healthcare regulations.
- Analyze data from various databases to track patient progress and outcomes.
- Educate patients and their families about health conditions, treatment options, and wellness strategies.
- Ensure adherence to safety protocols and infection control measures within the healthcare setting.
Requirements
- Current Registered Nurse (RN) license in the state of practice.
- Proficiency in medical terminology and familiarity with health information management systems.
- Strong data management skills with the ability to analyze complex information effectively.
- Experience in hospice care is preferred but not required.
- Excellent communication skills, both verbal and written, with a focus on patient education.
- Ability to work collaboratively within a multidisciplinary team environment.
- Strong attention to detail and organizational skills.
- IT department experience is required.
- Minimum Education - Bachelor's Degree
