RN Home Health Clinical Manager
We are seeking an experienced and highly motivated Clinical Manager to lead and oversee clinical operations in a home health setting.
Position Responsibilities:
- Supervised day-to-day clinical and office operations. Is available to staff and patients during hours of operation. Ensures services are in compliance with professional standards, agency policy, and state and federal regulatory requirements.
- Coordinates referrals. Coordinates patient care with an interdisciplinary team approach. Assures the development, implementation, and updating of the individualized plan of care. Evaluates the quality of clinical care, ensuring that patient needs are casually assessed.
- Assures clinicians including contract staff have required training, competency, and supervision. Makes patient and personnel assignments assuring that assigned staff have demonstrated competence in the required tasks/services. Ongoing coaching and mentoring of field clinicians.
- QAPI owner and participation in QA and performance improvement activities. Reviews/investigates incidents, analyzes for trends, implements corrective action plans as needed. Accreditation/Survey Readiness, QA audits/detailed corrective action plan audits. Manages HHCAHPS distribution in SHP. Emergency Preparedness Planning – Activation Reporting & Ongoing Event Monitoring and Reporting.
Required Skills & Qualifications:
- Bachelor’s Degree in nursing, healthcare administration, or a related field.
- Current nursing license or relevant clinical licensure.
- Home Health leadership experience and OASIS experience.
- QAPI/Survey Readiness (is a plus)
- Proven experience in a clinical managerial role or similar position.
- Strong understanding of healthcare regulations, compliance standards, and quality assurance.
- Excellent leadership, organizational, and communication skills.
- Ability to analyze patient care data and implement evidence-based practices.
- Proficient in electronic health record (EHR) systems and healthcare technology.
Job Type: Full-time
Pay: $105,000.00 - $120,000.00 per year
