What are the responsibilities and job description for the Veterans Case Manager position at New Visions Homeless Services?
Summary of Work: Under the supervision of the Site Director at the Omaha campus, the Veterans Campus Case manager will provide Housing-based case management services to veterans experiencing homelessness and staying at the Veterans campus in Omaha. The Veterans Campus Case Manager will work to identify, assess, and refer residents to appropriate services throughout the community with a focus of restoring, attaining, and maintaining public benefits, VA benefits, entitlements, and housing. The Case Manager will strive to build and maintain authentic relationships through consistent and solution focused engagement with all residents, while building connections between the agency and the Omaha/Council Bluffs community in a manner consistent with established protocols.
Duties and Responsibilities:
- Demonstrates knowledge of current community resources to meet the unique needs of veterans.
- Assist individuals in locating, attaining, and sustaining permanent housing.
- Develop and update Individual Service Plans and Safety Plans
- Consistent communication with peers, volunteers, and other support staff to ensure effective communication and transparency.
- Provide case management services focusing on financial management, household management, addressing physical and mental health needs, transportation, crisis planning/management, navigating community resources, community engagement, and developing and maintaining healthy support systems.
- Is prompt and accurate in completing all reports, statistics, and other requirements.
- Is dependable and punctual regarding scheduling and attendance.
- Assist clients through every step of housing placement: applications, housing paperwork, furniture and any other needs.
- Daily documentation, case notes, file management, and file update/ review, and follow- ups.
- Ensure residents abide by established program rules and ensure overall safety of the facility.
- Must be able to demonstrate the ability to work independently, and with a high degree of integrity.
- Meeting with supervisor and other staff on a regular basis to determine the problems/successes of program operations, as well as attending training and meetings as scheduled.
- Other duties may from time to time be assigned, which are consistent with these overall duties.

Qualifications & Experience:
- At least two years of experience in the social service field, preferably with individuals experiencing homelessness
- Capacity to multitask and work effectively in a fast-paced environment
- Strong computer proficiency, particularly in Microsoft Office
- Driver’s license required
- Meticulous level of organization and attention to detail
- Excellent verbal and written skills are required.
- Experience in the mental health or substance abuse field is desired
- Must provide reliable current references
Education: A bachelor’s degree in social work, Psychology, Sociology or a related field is desirable.
