The ACT Team Leader provides clinical leadership, oversight and coordinates administrative activities of the ACT Team. The Team Leader ensures the team is strategically and efficiently providing intensive holistic care to individuals with a serious mental illness (SMI) diagnosis benefiting from a multidisciplinary community-based outpatient care. The Team Leader collaborates with the team to ensure rapid assessment for individuals referred, up-to-date person-centered treatment planning and collaboration of providers to meet the needs of the individual from a whole health perspective. The ACT Team Leader provides direct supervision to ACT Team members as well as direct and indirect clinical support for individuals receiving ACT services. The ACT Team Leader is a positive team player that promotes a culture of inclusion and participation.
Required Education and Experience
- Master’s Degree social work, marriage and family therapy, professional counseling, psychology, or a closely related field that provides a specialized knowledge of the work to be performed
- License as required by Kentucky professional boards and billing sources
- Valid driver’s license, reliable transportation, and proof of current auto insurance
Preferred Education and Experience
- Experience in individuals with seriously mentally illness and/or co-occurring disorders
- Previous ACT experience

Administrative Operations
- Foster a collaborative approach among the ACT team and across service lines to provide the best care and create a seamless experience for each client embracing the “no wrong door” philosophy
- Ensure ACT provides a positive, professional and client centered, trauma-informed environment (in clinics and in the community) that is welcoming to clients, staff and community members
- Provide oversight and management of the ACT Team, directing day-to-day clinical operations
- Lead team meetings, staffing and meetings with community partners
- Provide supervision to ACT Team and ensure clients are receiving clinical care across disciplines that meets the individual needs and ACT fidelity expectations
- Monitor the team’s compliance with treatment standards as they pertain to the ACT model of service
- Manage referrals and ensure rapid assessment and acceptance and/or linkage to other resources, as appropriate
- Maintain structure for caseloads that balance client needs and efficient use of staff time and skills
- Monitor client risk effectively and ensure appropriate interventions are utilized among the team members to support the client’s unique needs
- Ensure all members of the individual’s care team are active members and part of the person-centered treatment plan and services
- Collaborate and coordinate with community partners, ensuring a positive, streamlined relationship with New Vista
- When appropriate, advocate with clients to ensure their individualized needs are met
- Maintain accurate, thorough, person-centered records on all assigned cases and use documentation to assist clients in evaluation of progress
- Ensure the team follows documentation guidelines and all charts are kept up-to-date and in line with New Vista policies
Non-Essential Functions
- Perform other related duties as assigned
- Participates in various committees as needed (for example, Olmstead Transition, Performance Improvement)
Supervisory Responsibility
This position will directly supervise various positions falling under the ACT Team department.
