The Housing Support Specialist is part of a comprehensive community mental health team, and in collaboration with persons served, is responsible for providing supportive services, crisis intervention, and expertise through comprehensive assessment, planning implementation, and overall evaluation of individual client needs. The overall goal of the position is to enhance the quality of client management and satisfaction and to promote continuity of care through the functions of recovery-oriented services. The Housing Support Specialist is a positive team player that promotes a culture of inclusion and participation.
Required Education and Experience
- Bachelor’s degree from an accredited college or university with a concentration in social work, psychology, or a closely related field that provides a specialized knowledge of the work to be performed
- Must have a valid driver’s license, reliable operating transportation, and proof of current auto insurance
Essential Functions

- Coordinate and provide case management for clients living in permanent supportive housing
- Serve as the primary liaison between case management and property management staff located at apartment sites throughout the Bluegrass Region
- Collaborate with case managers for housed clients to ensure continuity of care and access to local resources
- Provide case management in line with Housing First principles to work toward client-identified goals which may include everyday life skills such as financial management, household and self-care, learning to use public transportation, coordinating telehealth or transportation to relevant medical and psychiatric appointments and building natural family, friend and community supports to increase community engagement
- Assist clients in meeting requirements of their rental lease
- Responsible for providing clinical and programmatic documentation in accordance with New Vista standards
- Provide crisis intervention support and intervention in a compassionate and professional manner
- Communicate immediately with supervisor and other team member regarding concerns or changes in an individual’s appearance, mood, clinical or medical status
- Participate in team and treatment planning meetings to assist clients with identifying and attaining goals
- Maintain all appointments with clients
