Duties include :

• Providing crisis intervention, service linkages, and discharge planning
• Developing treatment plans with individuals and treatment team; monitoring and evaluating progress; and modifying treatment plans as needed
• Determining discharge needs and appropriate community resources
• Establishing and maintaining relationships with community providers to ensure continuity of careNOTE : 1.
