Duties Description

Duties include:

• Providing crisis intervention, service linkages, and discharge planning
• Developing treatment plans with individuals and treatment team; monitoring and evaluating progress; and modifying treatment plans as needed
• Determining discharge needs and appropriate community resources
• Establishing and maintaining relationships with community providers to ensure continuity of care

Central New York Psychiatric Center- Eastern Satellite Unit is recruiting for Licensed Master Social Worker to provide professional social work services to individuals with mental illness. Candidates from outside or within State Service can apply.

Minimum Qualifications:

Candidates from outside or within State Service can be considered for hire under the Hiring for Emergency Limited Placement – Statewide (NY HELPS) program if they are licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW).
