The New York State Troopers Police Benevolent Association, a statewide labor union, is seeking a full-time Care Coordinator to join our team in Albany, N.Y The Care Coordinator assists PBA Members (NYS Troopers, Retirees and immediate Family Members) in the coordination of care through the State Trooper Surgeon Executive Care Program, in partnership with many of the major medical institutions and nearly 3,500 individual physician members throughout New York State.

The candidate will work in the non-clinical role of Member Advocacy and Healthcare Navigation. As a “Healthcare Navigator”, the candidate’s primary job is to remove any barriers to appropriate care that members encounter and provide options and education for the members to consider. Once members provide a medical need, the role then changes to opening the most appropriate access to appointments and following to full medical resolution. The candidate will NOT treat or diagnose with respect to this position. This is an advocacy role that requires verbal member connection and the management of relationships with partner hospitals, executive care liaisons, PBA Chief Surgeons, physicians and others involved in access to care.

The candidate must be a compassionate listener with basic knowledge of numerous medical specialties and disease states. Those with a background in Medical Administration/Healthcare Navigation will be given priority. The candidate must be willing to demonstrate empathy and respect and be prepared to work through any issue that may prevent access to the best care.

A working knowledge or aptitude for business communication tools such as Word, Excel, Teams, Outlook as well as web searching and form use is essential to the success of the candidate. Training on accessing our network of providers will be provided. Administrative duties and assistance with the overall Surgeons Group Program will be required.

The position is hourly and is expected to be an 8 AM – 4 PM, Monday through Friday schedule. This position will require occasional travel and overnights for meetings and/or events. A 40-hour work week will be expected, and the candidate will need access to reliable transportation and be willing and able to be present at 120 State Street, Albany, NY for all work duties, as this is not a remote position. Once a candidate has been presented an offer for employment and it is accepted, they will be subject to a background check.

Education and Experience:

  • Required: Bachelor’s degree in a human service-related field with 2 years of work experience providing case management/care coordination services.
  • 5 years of healthcare experience is required.
  • Excellent communication and interpersonal skills.

  • Experience in case management and providing emotional support to patients and families.

  • Strong written and verbal communication skills; Provides excellent customer service and demonstrates a high level of cultural competency. Has the ability to adhere to strict confidentiality guidelines. Have a working knowledge of computers/technology.

Key Responsibilities:

1. Understand current insurance held by all PBA Members as well as Workers Compensation.

2. Understand relationships with partner hospitals, appointed chief and deputy chief surgeons, State Trooper Surgeon members, liaisons and the manner used to present medical options.

3. Accept phone calls and emails from PBA members, complete or direct a Medical Intake Form.

4. Understand and fully utilize the medical intake form for both emergent and non-emergent medical requests.

5. Understand and fully utilize the list of State Trooper Surgeon members, emailing and/or calling to coordinate care.

6. Work in conjunction with partner hospitals and medical groups to administrate access for PBA members to preventative health screenings, such as heart scans, skin cancer, podiatry, and other screening opportunities as they arise. The Director will coordinate events, and the Executive Care Coordinator will handle signups from start to finish.

7. Administrative duties to include:

a. Member care tracking and report writing via Teams, Outlook, Excel, Word and other software solutions.

b. Email program usage surveys as well as healthcare screening surveys, when applicable.

c. Generate reports and presentations for monthly staff meetings

d. Handle all mailings: Mail get well soon cards, holiday cards, yearly membership invoices, additional package item requests, returned mail items and marketing materials. Assist Director in yearly physician membership package mailings.

e. Handle all physician membership updates, such as address and practice changes, etc

f. New physician application medical background checks

8. Must be willing to sign an NDA, as it protects the physician membership data.

9. Maintain HIPPA and any other necessary certifications, work to ensure that all member documentation is protected and meets HIPPA criteria.

10. All other duties that may be assigned as required by the Director of Medical Operations or Office Manager.

Job Type: Full-time

Pay: $53,000.00 - $62,000.00 per year
