What are the responsibilities and job description for the Behavioral Health Outreach Worker position at Newark Community Health Centers, Inc.?
The incumbent in this position is responsible for conducting and facilitating community outreach efforts geared to the behavioral health population within the community. Working in conjunction with leadership of other social services programs within the organization to identify. The Behavioral Community Health Worker interfaces with social services programs to provide linkages to a wide variety of resources. In addition to measure success, the Community Health Worker will have access to all internal and external resources, as is practical. This job may have routine access to individual health information including the patients’ entire medical records on a need-to-know basis only. Essential Duties/Responsibilities:
- Works in tandem with the clinical team – Doctor, Nurses, etc., is responsible for meeting the patient psychological needs.
- Ensures the patients right to privacy and ensures appropriate confidentiality when, information about the patient is released/ shared with others.
- With the guidance of the Director of Clinical Programs, develops, organizes and coordinates psychosocial, and health enhancement activities for a high-risk population of patients.
- Conducts community and street base outreach services in order to improve patient compliance with their health care needs.
- Facilitates bimonthly social/support groups.
- Develops and implements programming activities to meet all program goals.
- Provides client advocacy with medical providers, law enforcement agencies, social services agency and other institutions as needed.
- Ensures that all work done with patient or on their behalf is documented in adherence with agency policies on documentation.
- Participates in program team building, case conferences and training meetings.
