We are currently looking for a MDS/ MMQ/ Case Manager who is responsible for ensuring the development and completion of the resident assessment in accordance with the requirements of MA state and the policies and goals of the facility.
Key Responsibilities:
- Completes assessments, Minimum Data Set (MDS} and care plans for all residents assigned.
- Develop preliminary and comprehensive assessments of the nursing needs of each resident, utilizing the forms required by current rules or regulations and facility policies.
- Monitors completion of MDS & MMQ by other disciplines within timeframes prescribed by regulatory guidelines.
- Advises supervisor of incomplete and/or untimely assessments by disciplines other than nursing.
- Complete all Admission, Conversion, Significant Change and Semi-Annual MMQs per timeframes outlined in MMQ instructions as updated by the Office of Long Term Care.
- Initiates care plans and supporting activities that will result in best possible outcome for assigned residents.
- Participates in morning clinical meeting
- Conducts Utilization Review meeting. Ensures that all disciplines have updated and accurate information.
- Participates in discharge planning, care plan meetings and family meetings
- Maintains and updates all care plans and assessments for assigned residents on a quarterly basis (at minimum) and adds/deletes issues as necessary.
- Reviews all resident incidents and accidents and the 24-hour report daily to ensure that care plans for assigned residents reflect current and changing needs.
Qualifications:
- Comprehensive knowledge of modern nursing techniques, routines and methods as applied to skilled care and of state and federal resident care regulations.
- Ability to make independent decisions on a regular basis.
- Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long term care.
- Ability to encourage a proper professional attitude among the members of the nursing staff.
- Ability to prepare accurate and detailed records and reports.
- Ability to effectively present information to customers, clients, and other employees of the organization
