Job Title: Licensed Social Worker
Location: Wayne, NJ
Job Type: Contract
Pay rate: $44.64 per hour on W2.
Newkleo is hiring a Licensed Social Worker for one of their clients in Wayne, NJ.
Job Description:
- Perform resident assessments.
- Provide resident-centered care as a member of the interdisciplinary care team.
- Plan for resident discharges and maintain required documentation.
- Provide counseling and guidance to residents and their families.
- Provides information on outreach services offered to families.
- Ability to establish and maintain cooperative relationships with clients and staff.
- Long-term care experience Social Worker in a nursing home facility. Must be able to assess/document in MDS.
Certification required:
- LSW, MSW.
Working Hours:
- 9am-5pm or 10am-6pm
- Lunch period: 1/2 hour Unpaid
