WHAT YOU WILL BE DOING (ESSENTIAL DUTIES) …
We are seeking an experienced Case Manager to join our Intensive In-Home Child and Adolescent Psychiatric Services program (IICAPS). The Case Manager will play a crucial role in providing home-based clinical case management to children with psychiatric illnesses and their families.
Main Duties and Responsibilities:
- Provide home-based case management services to children and their families.
- Participate in daily team meetings to review client visits, concerns, and coordinate care.
- Co-facilitate family sessions and collaborate on clinical treatment goals with families.
- Work toward stabilizing and maintaining children in their homes and communities.
- Develop strength-based comprehensive assessments and treatment plans.
- Apply model-specific concepts to aid clients in understanding and managing their symptoms.
- Maintain family and collateral contacts to ensure quality, client-centered services.
- Assess and involve natural supports in treatment planning and discharge processes.
- Provide case management, clinical, and crisis intervention supports as indicated, collaborating closely with assigned clinicians during crises.
- Complete self-administered urinalyses on a weekly basis if required by the model.
- Demonstrate competency in child development, substance use, and provide trauma-sensitive and culturally competent care.
- Utilize Electronic Health Records (EHR) to document all services, client and collateral contacts, and appointments.
- Attend all model-required meetings and agency trainings.
WHAT WE EXPECT OF YOU…
- Minimum of 2 years of experience in the Human Services field.
- Bachelor’s degree in a related field (i.e psychology, social service, counseling with an expressed interest in continuing in the human service field).
- Exceptional verbal and written communication skills to engage with clients and stakeholders effectively.
- Self-motivated and independent, with the ability to prioritize tasks and meet deadlines.
