Health Promotion Advocate – Maternal and Child Integrated Mental Health Program –Columbia - Days -Grant Funded
New York-Presbyterian Hospital/Columbia University Medical Center offers integrated mental health services for adults, children and families receiving primary care in our Ambulatory Care Network clinics. Many patients have complex biopsychosocial needs and require support in navigating social needs and systems. This grant funded position will enable the program to extend the coordination and support for families with young children with developmental, behavioral, social, and emotional needs, to reduce barriers to care and enhance integration.
Within the ACN, NYP launched the Maternal and Child Integrated Mental Health Program (MAC-IMP) to bring together three key departments treating women and children — Pediatrics, Behavioral Health), and Obstetrics — forming an interdisciplinary team and providing a coordinated, dyadic, two-generational approach.
An essential part of MAC IMP, the Health Promotion Advocate will join our multi-disciplinary team to support patient screenings and follow-ups, connect patients to community-based resources such as WIC, WSCAH, NMIC and others to address social needs. Seeking candidates with strong patient engagement skills, efficient organizational skills, data entry, and investment in teamwork.
Preferred Criteria
- Fluent in Spanish
- Experience in a community healthcare setting
- Bachelor's degree
Required Criteria
- High School diploma or GED
- Diverse and culturally sensitive customer service skills
- Microsoft Word and Excel skills
- Ability to manage high stress, chronically ill population with complicated psycho-social issues
