Patient Navigator – Robert Center for Community Health Navigation – Full-Time – Evenings - Queens
The Patient Navigator serves as the primary care coordinator for patients, working closely with key clinical staff to ensure comprehensive care and treatment. This role involves:
- Care Coordination: Scheduling and managing patient appointments, providing logistical support, and facilitating communication between patients and the medical team.
- Patient Support: Addressing patient inquiries, directing them to the appropriate staff, and assisting with the collection of medical records, insurance details, and financial information.
- Communication: Engaging with patients in person, over the phone, and via email to ensure seamless coordination of care.
The Patient Navigator Program is based in the Emergency Department, where it serves both pediatric and adult populations, ensuring that each patient’s journey through healthcare is as smooth and informed as possible. You’ll play a crucial role in enhancing patient experience and access to care.
This is a full-time position based out of our NYP/Queens site. The shift is 3:00pm-11:00pm with alternating weekends required.
Preferred Criteria:
- Bachelor's degree
- 2-3 years of experience in primary or community health
- Familiarity with hospital operations hospital and community resources
- Fluency in Mandarin, Cantonese, or Spanish
Required Criteria:
- High School Diploma or GED
- Previous experience in healthcare
- Understanding of medical office practices and procedures
- Strong organizational, multi-tasking, and time management skills
- Flexibility, persistence, and patience in a dynamic environment
- Excellent oral, written communication, and interpersonal skills
- Proficiency in Microsoft Word and Excel

Please note that all roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP.
