Senior Social Worker - School Based Health Center Columbia ACN

Participates with interdisciplinary team members in developing and implementing program policies and procedures.
Assesses the social and emotional needs of patients. Provides direct counseling / social services to students referred by self, health team, faculty, administration and other school ancillary staff. Provides group education in clinic or classroom on mental health / social concerns of middle school students.
Tracks and follows-up on all referrals made in and out of school.
Coordinates social work services and patient referrals between the program and other agencies providing social and educational services in both the school and local community.
Establishes referral network / linkages with outside agencies for both students and parents.
Serves as an advocate for patients' rights within the clinic and serves as a mediator between parents, child and school.
Remains available for crisis intervention social work as the need arises or arranges for such coverage by emergency social services.
Provides clinical supervision of social work staff.
Acts as liaison between schools supervisor of SW, CHEP and other administrative staff.
Coordinates social work services on site.
Consults with teachers / administrators on student problems in the classroom. Provides information on psycho-social issues related to class behavior, especially that relate to health concerns.
Records professional assessments, identified problems, treatment plans and progress notes as required by the Department of Social Work Services and the Community Health and Education Program.
Acts as main liaison between clinic and school administration.
Is competent to care for patients based upon the age groups treated.
This is a Grant Funded 1199 position
Preferred Criteria :
- At least 1 year of experience in behavioral health crisis intervention
- Experience in with Child Mental Health
- Fluency in Spanish

52.02-$56.27 / Hourly
