Care Coordinator Assistant - LPN - Days
Under the guidance of a Care Coordinator, the Care Coordinator Assistant is responsible for concurrent and retrospective chart reviews. The Care Coordinator Assistant supports the leadership and direction provided by the Care Coordinator and supports projects for the department in conjunction with other leaders.
Required Criteria
- Current licensure and registration as a Licensed Practical Nurse in New York State
- Five years clinical experience
- Basic computer skills
