Medical Social Worker – Part-time position (3 days/week), 9-5 pm.  - BMH

The schedule will be every other weekend, plus various days during the week, TBD,  some flexibility.  This is a 1199 position.

The role of the Medical Social Worker is to work with the interdisciplinary team to ensure a safe discharge of all adult patients.  Duties include but are not limited to:

·         Identifies patients with barriers to a safe discharge plan.

·         Completes a psychosocial assessments on all patients.

·         Documents all clinical assessments, notes, and referrals in Epic.

·         Completes screens and coordinates all new skilled nursing facility and rehab facility referrals.

·         Assesses all cases involving child abuse/neglect, domestic violence, elderly abuse, and sexual assault.

·         Educates patient/family about post-acute care options and the role of choice.

·         Educates patient/family about managed care and payor issues 

 Preferred Criteria:

• At least one year of inpatient Medical/Surgical experience

• Discharge planning experience

• Experience with Epic and Allscripts

Requirements:

• Master’s Degree in Social Work (MSW)

• New York State License as a Licensed Masters Social Worker (LMSW)
