This position assists the Social Work Department in meeting the needs of the residents and their families to support them in dealing with the impact of illness and extended care placement. This position is also responsible for assisting with admissions, and ensuring that available rooms are clean, neat and orderly. The Discharge Planner will meet and greet the resident and family upon admission to make sure the resident is comfortable. Daily responsibilities include visiting short stay residents to confirm their needs are being met, and assisting with the timely completion of Ready Q surveys.

What you will need to be a part of our EPIC team: :
- High School Diploma, or equivalent.
- 1-2 years of experience in a skilled nursing facility.
- Excellent computer skills; experience with electronic charting a plus.
- Must be able to multi-task and work in a fast-paced environment with little supervision.
- Excellent English written and verbal communication skills, including demonstrated ability to communicate effectively with residents, physicians, visitors and all facility personnel.
- Proven ability to relate appropriately and effectively to residents and their families and respond to their needs.
- A working knowledge of the basic services provided by skilled nursing and related facilities.
- Ability to handle confidential data discreetly.
How you will help people every day:
- Coordinating discharge planning services, i.e., helping to place a resident on a waiting list for community congregate living, arranging intake for home care services for residents returning home, assisting with transfer arrangements to other facilities.
- Identify the medically related social service needs of residents and assist with ensuring those needs are met by the appropriate discipline.
- Maintaining contact with family (with resident’s permission) to report on changes in behavior, psychosocial functioning, discharge planning, and encouragement to participate in care planning.
- Assisting residents/responsible parties with financial and legal matters such as drafting petitions for guardianship/conservatorship, assisting with pre-paid funeral arrangements, etc.
- Through the assessment and care planning process, identifying and seeking ways to support residents’ individual needs and preferences, customary routines, concerns and choices.
