Job Title: Licensed Practical Nurse (LPN) Long-Term Care (LTC)
Location: Kane, PA
Shift Type: Nights
Shift Schedule: 6:00 PM 6:00 AM (12-hour shifts)
Under the direction of the RN Supervisor, the LPN Wing Supervisor is responsible for carrying out medical care plans, overseeing nursing assistants, and ensuring compliance with state and federal regulations. Shift Type: Nights
Shift Schedule: 6:00 PM 6:00 AM (12-hour shifts)
- Two days on, two days off, three days on (rotating schedule)
- Night Shift Differential: $0.75 (starts at the beginning of the shift)

Key Responsibilities:
Clinical Duties:
- Administer and record medications, treatments, and glucose monitoring as ordered.
- Accurately transcribe physician orders and complete monthly summaries.
- Ensure care plans reflect the residents current condition and guide the nursing assistants in care implementation.
- Notify the RN Supervisor of any significant changes in a residents condition.
- Communicate resident transfers to immediate family and department heads.
- Assist physicians with rounds, examinations, and treatments.
- Collect laboratory specimens (urine, cultures, stool, etc.) and document appropriately.
- Oversee and assist with post-mortem procedures when necessary.
- Encourage resident participation in scheduled activities.
- Supervise and direct nursing assistants to ensure adherence to care plans.
- Assist with annual evaluations of nursing assistants and provide feedback.
- Adjust staffing assignments to ensure quality resident care.
- Conduct team conferences and monitor policy/procedure changes.
- Ensure accurate documentation of incident reports, narcotic counts, and weight discrepancies.
- Monitor infection control compliance and participate in quality assurance studies.
- Maintain accurate resident medical records in Physicians Computer Company (PCC) EHR/EMR system.
- Ensure proper storage and disposal of medications and compliance with regulatory requirements.
- Maintain inventory of resident belongings and ensure a clean, safe environment.
- Submit maintenance requests and assist in general housekeeping duties when needed.
- Attend care plan conferences, in-service training, and staff meetings as required.
