Nexus-FACTS is excited to add fully-licensed, on-call Mental Health Professionals for its innovative Family Response and Stabilization Services (FRSS) program! This program crisis provides response and stabilization services to Hennepin County families with youth ages 0-18 everyday, 365 days a year!
Crisis need is defined by the family and services are driven by family’s cultural, community and clinical needs. Our Family Response Team de-escalates and addresses the immediate concern with the goal of responding in-home within one hour. Our Family Response Team connects family with other supports with option of eight-week stabilization services to help promote well-being of the family.

Schedule/Pay
- On call, remote
- Stipend for on call week: $800/week
- On call rotation one week every 6-8 weeks: 11pm-7am Monday through Friday and all day Saturday and Sunday

- Coordinate the Intake Process
- Completion of required intake process and forms
- Development and completion of the diagnostic assessment or crisis assessment depending on the Agency
- Completion of clinical documentation in compliance with program expectations
- Develop and Implement Treatment Plans
- Development and implementation of the initial treatment plan based on the diagnostic assessment or crisis
- Treatment Plan Reviews, at least every 90 days
- Crisis intervention and safety planning as needed
- Provide Direct Therapy and Skills Services
- Provision of individual, group and/or family therapy as prescribed by the Treatment Plan goals
- Provide therapeutic services within the parameters of the program’s structure, philosophy, and models
- Meet the minimum required hours of direct client contact and collateral services to meet billable expectations
- Manage all aspects of Therapeutic Services
- Coordination of referrals for adjunct services when applicable and done in coordination with the case
- Provision of case coordination with family members, other professionals, school staff and community support
- Attendance and participation in mandatory clinical supervision and team case consultation meetings in
- Completion of education and training experiences in accordance with program expectations.
- Compliance with all agency, state, and regulatory standards.
- All other tasks and duties as assigned.
- Master’s degree in social work, psychology, or clinically related field required
- Licensure within related clinical field (LICSW, LPCC or LMFT) per state board requirements required
- Valid driver’s license required
- Experience in related community-based work with youth, adults, and/or families
