Summary
Nexus Health Resources is seeking a dedicated and compassionate individual to join our team as a Bilingual Fluent in Spanish and English - Transition Care Coordinator and Patient Experience Surveyor. In this role, you will be responsible for conducting surveys by phone in both English and Spanish. Additionally, you will perform check-in phone calls in both English and Spanish to patients recently discharged from hospitals or nursing homes, ensuring their well-being, and providing assistance as needed. Communication with patient care team members to relay patient concerns is also a crucial aspect of this position. The successful candidate will report to the Nexus Health Call Center Director.
Responsibilities:
- Conduct thorough surveys in both English and Spanish to collect patient feedback and satisfaction data.
- Perform supportive check-in calls in both English and Spanish for recently discharged patients, addressing concerns and providing assistance.
- Effectively communicating with the patient's care team, promptly relaying concerns and feedback.
- Maintain accurate documentation of survey responses, patient interactions, and care team communications.
- Collaborate with colleagues and Nexus Health Call Center Director for streamlined patient care.
- Proactively identify and address issues during patient interactions, escalating when necessary.
- Adhere to protocols and guidelines for surveys, patient interactions, and compliance with healthcare regulations.
- Participate in ongoing training for industry best practices and contribute to continuous improvement efforts.
- Provide exceptional survey and call experience; demonstrating empathy and active listening.
Qualifications
- Fluent in Spanish (reading and speaking)
- Fluent in English (reading, speaking, and writing)
- Strong verbal and written communication skills
- Enthusiasm for spending a significant amount of time talking on the phone
- Proficient in using software
- Organized with excellent follow-through abilities
- Proactive in asking for assistance
- Demonstrates a strong sense of responsibility
- Sets high expectations for quality
