Job Overview: The Patient Care Coordinator (PCC) will be responsible for managing various aspects of patient
care coordination, including appointment scheduling, patient communication, insurance verification, coordination
with community partners, scheduling provider routes, requesting medical records, and assisting with provider
requests. This role requires strong communication skills, multitasking abilities, and a commitment to delivering
excellent patient service.
Responsibilities:
1. Appointment Scheduling:
- Efficiently schedule and manage appointments for patients, ensuring optimal utilization of provider time.
- Coordinate with providers and support staff to create balanced and effective schedules.
2. Patient Communication:
- Serve as a primary point of contact for patients, providing clear and timely information about appointments, procedures, and any necessary preparations.
- Address patient inquiries and concerns, ensuring a positive patient experience.
3. Insurance Verification:
- Verify patient insurance information to ensure accurate billing and claims processing.
- Work closely with the billing department to resolve any insurance-related issues.
4. Community Partnerships:
- Collaborate with community partners to enhance patient services and ensure a seamless healthcare experience.
- Coordinate and communicate with external organizations to facilitate comprehensive patient care.
5. Provider Route Management:
- Schedule and optimize provider routes for efficient time management, particularly for traveling providers.
- Coordinate with providers to accommodate their preferences and streamline route planning.
6. Medical Records Management:
- Request and obtain medical records from various sources to ensure comprehensive patient care.
- Maintain organized and secure records, adhering to privacy and confidentiality regulations.
7. Assistance with Provider Requests:

Qualifications:
- High school diploma or equivalent; additional education in healthcare administration is a plus.
- Proven experience in a healthcare or medical office setting, preferably in a patient care coordination role.
- Strong organizational skills with the ability to multitask and prioritize tasks effectively.
- Excellent communication skills, both written and verbal, with a focus on patient-centered communication.
- Proficiency in using electronic health records (EHR) and scheduling software.
- Knowledge of medical terminology, insurance processes, and HIPAA regulations.
