NFI Massachusetts, Inc., a non-profit human services organization, is currently seeking a full-time Case Manager for its residential youth program located in Haverhill, MA working with youth 17-22 involved with the Department of Children and Families.
The Case Manager directly assists the clinicians with a variety of functions. This includes but is not limited to scheduling of treatment team meetings, communication with school districts and contact which includes family members and collateral case workers, case-coordination, documentation, data collection. The Case Manager also may support the residential staff in the milieu during activities or other aspects of the daily structure when requested.
Job Responsibilities:
- Support clinical and management staff with administrative tasks as assigned.
- Clinical case management; data entry and collateral communication for program youth
- Participate in day-to-day program structure including groups/activities when possible.
- Provide documentation for weekly TEAM meetings.
- Participate in community meetings.
- Available for crisis intervention and support in the milieu.
- Perform physical intervention in the event of a youth crisis.
- Perform all duties in accordance with the Agency’s policies and procedures.
- Support the administrative team with 24 hour on call coverage
Job Requirements:
- Experience with youth population required.
- Bachelor’s degree in related field required.
- High energy level, superior interpersonal skills and ability to function in a team atmosphere.
- Must have excellent organizational skills.
- Good communication skills.
- Must hold a valid driver's license.
Salary: $55,000-$57,000
