NFI North’s Early Childhood Wraparound Program is seeking Wraparound Facilitators/Care Coordinators for the State of NH. While our main office for this work is in Littleton, we also have satellite offices in Portsmouth and Tilton.
This position provides family-driven planning and facilitation utilizing the New Hampshire Wraparound Model. In this model the family is in the driver’s seat with their voices being at the forefront of the Plan of Care. We help the family identify strengths and unmet needs of the child and family/primary caregiver(s), obtain support services that are specific to family needs, and monitor the provision and outcomes of the plan so that the child and family can be successful at home, at school and in the community.
This program emphasizes family and infant/young child driven services. To be effective, the Wraparound Facilitators/Care Coordinators work with families of infants and young children in their homes and communities. They incorporate the entire family and their natural community supports in the planning process, focus on the child's and family's strengths, and demonstrate cultural and linguistic competence.
This position requires a BA in Social Work, Psychology or related field in addition to experience working with infants, young children and families. Applicants must be willing to travel mainly within the state of NH to support infants, young children and their families.
