NFI North is seeking a full-time Clinical Case Manager for our (2) five bed Residential Treatment Programs that specialize in providing the supported housing services to individuals transitioning from and/or awaiting admission to Glencliff Home. These (2) program sites are located in Newport New Hampshire. These new programs will augment our existing continuum of care in large part due to our continued growth in serving this valuable population in NH. The Agency has a progressive reputation of providing comprehensive clinical, nursing, medical, case management, vocational, educational and peer support services and life skills to adults living with Serious Mental Illness (SMI) or Serious and Persistent Mental Illness (SPMI) and/or complex medical needs. The participants residing in these treatment facilities are seeking independent home-like living.
Responsibilities: Provide case management and clinical services to assigned participants, administrative duties and clinical support for participants, including where applicable supported employment as well as providing clinical training and support for staff. Responsible for all financial information and benefits management.
Qualifications: Bachelor’s Degree in Human Service field with four years related experience and/or training. Experience with supported employment activities and the participant population preferred.
