Position : Clinical Social Worker - Full Time
NHC Springfield is interviewing for a clinical social worker and the qualified applicant should hold a bachelor's or master's degree in social work or a degree in sociology or psychology or related humanities.
Experience preferred with a case management background in a SNF / ICF or acute care setting, has knowledge of MDS, clinical assessments and documentation, care planning, discharge planning, working with support agencies such as hospice, home health and mental health services, and 1 : 1 case management with patients and their support systems.
Responsibilities include, but are not limited, to :
- Exhibiting strong clinical and customer service skills in a fast-paced healthcare environment
- Working with individual patients and their representatives toward transition back to the community or adjustment to center life
- Helping navigate a patient's crisis of illness and / or disability
- Serving in an advocacy role for patients and their representatives in expressing and resolving grievances
- Coordinating patient's plan of care with interdisciplinary team members
- Reporting to the Director of Social Work current case management status and needs
- Maintaining confidentiality of professional information
