What are the responsibilities and job description for the On-Call LPN or RN - Per Diem position at Niagara Hospice?
Niagara Hospice is currently seeking an On-Call LPN or RN to join our Home Care Team on a Per Diem basis for overnight shifts (4 : 30pm - 8 : 30am).
The on call LPN or RN is responsible for providing hospice nursing services to Niagara Hospice patients and their family members during on call shifts to include comprehensive direct patient care, indirect care, and psychological support.
Job Responsibilities :
- Evaluate the patient / family’s immediate problems and needs during a nursing visit.
- Performs direct patient care when indicated. This includes personal care, vital signs, administration of medications, treatments, and any other procedures within their professional scope following agency’s policy and procedures.
- Facilitates changes and / or addition to physician orders upon receipt of MD order.
- Teaches the families basic nursing skills, as well as signs and symptoms of disease progression as the need arises.
- Coordinate patient transfer for acute care / palliation, or respite care.
- Educates patients / families on palliative and end-of-life care and issues in a proactive manner.
- Assists physicians, co-workers, and families as needed in time of stress or crises.
- Charts information in the patient’s EMR & communicates findings with triage RN
- Monitors and orders narcotics and medications in accordance with State and Federal laws and regulations.
