Provides individuals and families with recommendations and options for services. Develops a person-centered plan of care based upon assessed needs. Attends care conferences, team meetings, and intermittent visits to the client for changes in needs and updates to services. Assures providers are delivering services as authorized. | Daily 20% | C4 |
Prepares and maintains client records that meet federal, state and local guidelines, including documentation of case management services, assessment activities, and correspondence with providers; ensures compliance with applicable protocols. | Daily 15% | C4 |
Interprets agency, health plans, and Department of Human Services policies for clients and families. Participates in professional development activities including attendance at trainings, webinars, and regional meetings . | Daily 5% | C4 |
Provides on-going case management to clients per the CBSM (Community Based Services Manual) and other DHS (Department of Human Services) policy for HCBS programs and MCOs. Face to face visits with clients/families/guardians at least twice a year | Daily 40% | C4 |
Maintains accounting of time using Social Services Information System (SSIS) and other administrative tasks, including staff meetings, staff training and clinical supervision. | Daily 5% | |

A bachelor’s degree in social work, nursing, or a closely related field, and two (2) years of home and community-based experience in assessment and care plan development; or,
An equivalent combination of education and experience sufficient to successfully perform the essential duties of the job.
Preferred Qualification
At least one (1) year of experience in education or treatment of people with developmental disabilities or related conditions.
Salary : $61,642 - $89,381
