Medical Social Worker, Home Health - part-time needed!

We are currently looking MSW part-time position to be able to provide service in Broward County!
The MSWs provide evaluation and treatment to patients having problems of social and personal adjustment that interfere with independent living or are a reaction to the medical diagnosis and are responsible for developing a treatment plan and providing individual, family or group therapy.
Duties Include:
- Provides psychosocial evaluation, diagnosis, treatment, and referral for patients and/or families. Evaluation shall include patient and/or family goals.
- Interprets results of evaluation and recommends treatment as indicated to the patient/family referral source.
- Provides services that include but are not restricted to community referral, individual therapy, family therapy, group therapy, consultation, or education.
- Aids the health care team in identifying, understanding and integrating the significance of psychosocial factors in relation to the patients illness, treatment, and recovery.
- Plans and leads individual and family therapy, patient and family group and leads family conferences.
- Provides a written report for patient medical record (evaluation, progress notes, and discharge summary) with copies to referral source, patient, and family when indicated.
- Maintains accurate, up-to-date records in the client chart.
- Consults with physicians, families, and referral source regarding clients progress and therapy program.
- Maintains all departmental records in accordance with companys policy, state and federal regulations.
- Maintains confidentiality of clients situation, records and services provided according to HIPPA regulations.
- Discusses with the Director of Clinical Services problems concerning the patients and how these may best be resolved, changes in patient needs and how these can be best met, and need for involvement of other members of health team.
- Coordinates total patient care.
- Masters degree in medical social work or social work license
- 2 years of social work experience in a medical setting, preferably in home health
- Current CPR Certification
- Valid drivers license and car insurance
- Ability to attend 1/2 day orientation at our Coral Springs office.
