Role Overview
As an Inpatient Nurse Practitioner at Nimbus Health, you will be at the forefront of delivering high-quality, patient-centered care to individuals with complex pulmonary conditions. This dynamic role involves:
- Inpatient Rounding (LTAC units, general medicine floors, emergency room)
- Discharge Day Management (coordinating safe transitions and preparing patients for successful return home)
- Transitional Care Management (post-discharge follow-up and cross-cover for readmission prevention)
You’ll work under our own renowned pulmonologist Dr. Jon Dean.
Key Responsibilities
1. Clinical Care
- Comprehensive Evaluations: Perform in-depth history-taking, physical exams, and diagnostic assessments for hospitalized patients with pulmonary and complex medical conditions.
- Treatment Planning: Order, interpret, and adjust treatment regimens—ensuring alignment with best-practice guidelines and patient-specific needs.
- Complex Case Management: Collaborate with on-call physicians for overnight pulmonary concerns, acute patient status changes, and urgent care decisions.
2. Discharge Day Management
- Safe Discharge Coordination: Work with physicians, nursing staff, and case managers to finalize discharge instructions, medication reconciliations, and follow-up appointments.
- Patient & Caregiver Education: Provide clear, concise guidance on treatment plans, medication schedules, warning signs, and lifestyle modifications.
- Documentation & Handoffs: Complete discharge summaries, update medical records, and ensure accurate information transfer to the next level of care.
3. Transitional Care Oversight
- Cross-Cover Calls & Admissions: Respond to patient needs, admission orders, and care transitions across ER, hospital floors, and LTAC.
- Post-Discharge Follow-Up: Participate in telehealth check-ins, coordinate home health services, and conduct outpatient clinic visits to monitor patient progress.
- Interdisciplinary Collaboration: Work with social workers, respiratory therapists, and other allied health professionals to address social determinants of health and arrange holistic support.
4. Collaborative Care & Quality Improvement
- Team Rounds: Attend and contribute to interdisciplinary meetings in LTAC, hospital units, and outpatient settings.
- Family & Caregiver Engagement: Provide empathetic communication, address concerns, and encourage active participation in the care process.
- Continuous Quality Improvement: Engage in internal QI initiatives to refine clinical protocols and enhance patient outcomes.
Required Qualifications
- Experience
- Minimum of 1 year(s) in internal medicine, ICU, hospitalist, or acute care setting.
- Proven ability to manage complex patients with sound clinical judgment.
- Education & Certification
- Master’s (or Bachelor’s with appropriate advanced practice certifications) from an accredited Nurse Practitioner program.
- Licensed as a Nurse Practitioner in Georgia with National Board Certification.
