Job Summary
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team. Responsibilities
- Provide direct patient care, including phlebotomizing and IV insertion as needed.
- Assist in the development and implementation of individualized care plans, focusing on acute pain management and discharge planning.
- Monitor patients' conditions and report any changes to the supervising registered nurse or physician.
- Perform diagnostic evaluations and maintain accurate patient records using Cerner or similar electronic health record systems.
- Administer medications and treatments as prescribed while adhering to established standards of care.
- Educate patients and their families about health conditions, treatment plans, and preventive measures related to infectious disease care.
- Collaborate with other healthcare professionals to ensure comprehensive patient care.
Qualifications
- Valid Licensed Practical Nurse (LPN) license in the state of practice.
- Experience in home community care or acute care settings is preferred.
- Proficiency in performing phlebotomy, IV insertion, and other nursing procedures.
- Strong understanding of discharge planning processes and infectious disease management.
- Familiarity with Cerner or similar electronic medical record systems is a plus.
- Excellent communication skills and the ability to work effectively within a team environment.
