Responsibilities:
- Plan, develop, organize, implement and direct the Social Service programs of the nursing home.
- Supervise and direct the employees of the Social Service Department.
- Assure that Care Plans and documentation are updated and accurate.
- Act as the liaison between the Interdepartmental Team and the Social Workers to communicate important information between them
