DCF Credentialed Services Case Manager
General Description of Position:
Case Manager is responsible for providing contracted services and monitoring the progress of clients admitted to our various programs:
- CHAP- Community Housing Assistance Program
- CHEER- Community Housing Employment Enrichment Program
- SS- Support Staff
- SV- Supervised Visitation
- TC- Temporary Care
- TSS- Therapeutic Support Staff
Major Duties and Responsibilities: (include, but not limited to)
- Assist with securing housing and employment within DCF established guidelines for the region
- Implement comprehensive treatment plans for successful young adult life skills progress
- Provide site-based services to facilitate contact between children and family members
- Collaborate with other providers towards the implementation of the child/youth individual treatment plan
- Able to multi-task and execute directives given by supervisor in an effective and productive manner
Monitor clients’ attendance and performance at educational and/or employment cite(s).
Provide ongoing support, consultation, and problem-solving assistance to client(s) a minimum of 10 hours per week for the first month and approximately 5 hours per week thereafter for a period of time determined by DCF or until client reaches the age of 23 (usually one to two years).
Complete assigned progress and collateral notes by determined due dates
Be available to assigned clients on an emergency or urgency basis. You are expected to carry a cellular phone to fulfill this responsibility. NCSS will provide a second phone line for your cellular.
Complete the LIST-Learning Inventory of Skills Training assessment and monitor/support client’s life skill progress and development in the LIST life skills domains.
Make periodic unannounced visits to the clients’ apartments to assess the clients’ living status and compliance with program expectations.
Mentor young clients with behavioral concerns and implement a life skills case management treatment plan
Assess, engage, and/or initiate child and parent interaction during supervised visitation hours deemed by DCF, and complete report(s) within 24 hours to supervisor.
Case Manager is supervised and reports to the Coordinator of DCF Credentialed Services
Qualifications:
CHAP/CHEER, SS, TC, TSS: A degree in Human Services or related field and a minimum of three years of experience or an Associate’s with five plus years working with at-risk youth and young adults ages 8 to 23.
SV: A degree in Human Services of related field is REQUIRED.
Familiarity with Case Management procedures, techniques, and trauma-informed care. Ability to effectively work with DCF Social Workers and other providers. Must hold a valid CT driver’s license, a roadworthy automobile, and must provide Agency with proof of suitable automotive insurance. The employee must pass a CPS/DCF background check, state police background check, and a DMV motor vehicle check to be qualified for credentialing.
