What are the responsibilities and job description for the Clinical Services Supervisor position at Noble Hospice and Palliative Care?
We are seeking a Clinical Services Supervisor to join our team! This candidate is willing and able to drive throughout the valley to meet the expectations of the position.
POSITION RESPONSIBILITES:
- Reviews and evaluates each case through a variety of means such as home visits, conferences, record review and the services provided by clinicians; discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services; and is available at all times during operating hours to assist clinicians as appropriate.
- Assigns appropriate hospice personnel to case as needed.
- Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.
- Attends case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.
- Assists in the screening and interviewing process of new employees and makes recommendations for employment of individuals.
- Participates in the orientation of all new employees.
- Assists Director of Clinical Services and other supervisory hospice personnel in the planning, implementation and evaluation of in-service and continuing education programs.
- Complies with accepted professional standards and principles.
- Assesses health needs of community and assists in the development of plan(s)/ program(s) to meet needs.
- Participates in public relations and community activities that promote the organization's role as an effective member of the hospice and health care delivery system.
- Participates actively in quality assessment performance improvement teams and activities.
- Collaborate with the Director of Clinical Services and the Director of Quality Assurance to provide one-on-one education to all skilled nurses on a regular basis.
- Oversees the hospice aide supervisory team as well as hospice aide annual education and competency requirements.
- Responsible for assisting with completing joint visits and annual competency assessments of the skilled nursing team.
- Must demonstrate excellent organizational skills and be available for phone calls, emails, and Teams chats for the duration of their shift.
- Responsible for completing nursing visits as requested by the Director of Clinical Services, which may include taking a caseload as needed.
- Performs other duties and activities as delegated by the Director of Clinical Services.

JOB CONDITIONS:
- Position may be stressful.
- On occasion, one may be required to bend, stoop, and move patient weight up to 250 pounds, lift and/or carry up to 30 pounds.
- Physical activities will include walking, sitting, stooping, and standing.
- Travel is required to and from patient’s homes when supervision of care is scheduled
- The ability to access patients’ homes which may not be routinely wheelchair accessible is required.
- Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient’s condition and to perform patient care.
- May require long periods of time in front of a computer.
- To fulfill the expectations of this position, the employee must be able to commute throughout the valley
EQUIPMENT OPERATION
Use of computer, cell phone telephone, calculator, copier and other office and medical equipment.
COMPANY INFORMATION
Has access to necessary patient medical records; ability to maintain compliance with HIPAA required. Must be able to keep information pertaining to personnel confidential.
QUALIFICATIONS:
- At least 18 years old.
- Current nursing license in Arizona.
- Graduate of NLN accredited school of nursing with a BSN degree is preferred.
- Previous experience in hospice/home care setting and with two (2) years management or supervisory experience preferred. Proven ability to work within an interdisciplinary setting.
- Complies with accepted professional standards and practice.
- Has excellent observation, good nursing judgment and communication skills.
- Understands hospice philosophy and issues of death/dying.
- Understands principles of pain/symptom management.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
- Complies and maintains current CPR certification.
- Travel is required to and from patients’ homes when supervision of care is scheduled.
- Must have the ability to communicate well both verbally and in writing.
- Must have the ability to demonstrate self-confidence and positive attitude toward self and others and maintain commitment and enthusiasm.
Job Type: Full-time
Pay: $110,000.00 per year
