What are the responsibilities and job description for the RN/LPN MAS NURSE position at Nobles Group Homes?
**MUST BE ABLE TO WORK AT LEAST 3 DAYS A WEEK DURING THE 8AM-5PM TIMEFRAME**
Duties:
- Conducts individualized assessments to include, but not limited to: Fall Risk Assessment, Nurses Assessment, Self-Medication Assessment, and AIMS Assessment as determined by the individual’s condition, recommended by physician, DMH/DD Standards, Nurses Delegation Program, and policies/procedures.
- Assure completion of appropriate lab work, referral appointments and follow-up appointments are held; annual routine examinations are done, i.e., including, but not limited to: physicals/standing orders, hearing, dental, and visual examinations. Request and obtain documentation from physician office in a timely manner.
- Assure appropriate follow-up as recommended by physicians for specialized healthcare, including, but not limited to: Occupational Therapy and Physical Therapy.
- Review all Record of Consults, prescriptions, physician’s orders, etc. for specific directions and report this information to all involved. Train direct care staff/others, as appropriate, with new orders affecting the care needs specific to an individual
- Conducts monitoring of group homes to ensure medication administration is performed according to MAC training and data collection is appropriately documented on medical forms. Visually observe individuals in their home setting.
- Reviews MARS, Narcotics, PRN Medications, Bowel and Bladder, Seizures, Blood Glucose, and etc. for issues/concerns and “red flags.”
- Inventorymedications received from pharmacy for appropriateness and follow-up with pharmacist in a timely manner regarding medications not received.
- Conducts MAC I and MAC II training for direct care staff/others per Nurse Delegation Program.
- Performs routine drug screen testing on newly hired staff and on a routine basis per Administrator, CEO, and Human Resource Manager.
- Provides individualized education customized to the individual and his/her family needs.
- Maintains confidentiality in matters related to resident, family and facility staff.
- Communicates with others and provides care in a non-judgmental, non-discriminatory manner that is sensitive to the individual and family’s diversity, preserving their autonomy, dignity and rights.
- Receives telephone calls during and after hours from staff seeking appropriate medical intervention and or medical advice.
- Performs all other responsibilities as mandated by the AL Nurses Delegation Program.
- Performs all other related duties as deemed necessary for the agency and individuals served.
