Program Summary : Our Houston Program serves individuals who are survivors of or at high risk for human trafficking, providing workforce training, mentorship, and case management to help them build a sustainable career path. As part of the developing Houston Program team, the Case Manager will also contribute to refining processes and best practices as needed.
Position Summary :
As the first Case Manager for the Houston program, you will play a foundational role in shaping and developing case management services. You will work closely with clients to assess their needs, create career plans, and connect them to essential resources while also contributing to the growth and refinement of program processes. This is an opportunity to build a program from the ground up and make a lasting impact on the community.
Location : Houston, Texas – Initially remote, transitioning to a hybrid schedule in South Central Houston
Reports to : Houston Program Manager
Schedule : M-F, full-time
Status : Exempt
Salary : $57,000-$61,000
Travel : Employee is expected to travel within the Houston area for events and partnership-related activities
Responsibilities :
- Establish and refine case management services as the first Case Manager in the program, helping to define best practices and develop processes for long-term success.
- Conduct comprehensive assessments with clients to understand their backgrounds, strengths, and barriers, and develop individualized career plans with both short-term and long-term education and employment goals.
- Collaborate closely with the Employment Specialist and program team to align job opportunities and career pathways with client needs.
- Build and maintain relationships with referral partners and community organizations to enhance client access to resources.
- Provide ongoing support and advocacy by offering resources, encouragement, and strategic guidance to help clients navigate challenges and stay on track with their education and employment plans.
- Develop and maintain relationships with referral partners and community organizations to expand client access to supportive services, including housing, mental health support, and recovery programs.
- Continuously assess and address employment barriers by connecting clients with tailored resources and external services to support their holistic well-being.
- Maintain accurate and detailed case notes and contribute to data collection efforts to measure impact and inform program improvements.
- Contribute to program development by identifying opportunities for process improvements, implementing feedback, and helping to shape the future of case management services as the program evolves.
