JOB SUMMARY:
The CHR - Transporter position will provide transportation to and from non-urgent medical appointments. Adhere to health care facilities to maintain confidential client information under Federal HIPAA and Indian Health Services quality and safety standards. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. In addition, the requirements listed below are representative of the knowledge, skill, and/or ability required.
MAJOR TASKS AND RESPONSIBILITIES:
- Responsible to provide courteous and prompt health care services to the Nooksack Indian Tribal Community.
- Responsible to document, maintain and complete daily transportation logs and forms.
- Responsible to report to supervisor and document any changes and/or updates, or cancellations to daily schedule.
- Responsible for maintaining clear and detail-oriented documentation of all client interactions and services.
- Participates in internal process improvements; such as staff meetings.
- Provides limited medical transportation services to Nooksack Tribal members without other means of travel to and from various sources of health care facilities for routine non-emergency appointments.
- Provides delivery services to tribal members for medications, medical supplies, and equipment when requested.
- Provides transportation for Tribal WIC Clients to and from clinic.
- Responsible to comply with all safety rules and regulations, follows proper safety procedures and utilizes proper protective equipment (PPE) at all times.
- Must complete and turn in all billing documents for transportation services performed on a daily basis to the CHR Support Specialist.
- Ability to work closely and coordinate with Nooksack Clinic Providers and Community Health Nurse to assure timely referrals, follow-ups, and participation as needed in preventative clinics.
OTHER DUTIES:
Because of the Tribe's commitment to community service and the well-being of its members, each employee may be expected to perform a wide range of office and field duties from time to time. Such duties may or may not be related to their regular responsibilities.

The following qualifications are required for the incumbent to have, in order to be considered for the position.
REQUIRED EDUCATION, EXPERIENCE AND TRAINING FOR POSITION:
High School Diploma or GED required.REQUIRED SKILLS/KNOWLEDGE/ABILITIES POSITION:
- Must be computer literate with specific working knowledge in Microsoft Office.
- Must be able to communicate clearly, effectively and courteously with Nooksack Health Department Staff and supervisor.
- Must continuously maintain a courteous and professional rapport with the clients and staff of the Nooksack Indian Tribal community.
- Must become or be familiar with the Nooksack Indian Tribal community health departments and resources available for patients, clients and their families.
- Must be respectful and sensitive to the values and culture of the Nooksack Indian Tribal Community.
- Must be caring, empathetic and dependable.
- Must be able to work independently with little supervision.
- Strong decision making and practical skills.
- Ability to communicate both verbally and in writing to assist client at medical appointment in completing paperwork for appointments.
- Knowledgeable about Nooksack Indian Community members, location of homes and be familiar with family names.
- Ability to act as an advocate for community members without consideration of family background or their personal habits. Must adhere to Federal Privacy Act and comply with Federal HIPAA regulations.
REQUIRED CONDITIONS OF EMPLOYMENT:
- Must pass alcohol/drug test at time of hire and throughout employment. Must pass criminal background check at time of hire and periodically thereafter.
REQUIRED LICENSES OR CERTIFICATIONS:
- Must obtain a CHR Certification, per the CHR policy, within the first year of employment.
- Must have and maintain throughout employment a valid Washington State Driver’s License and meet the insurability requirements of the Tribe. ( Must Provide Driving Abstract at time of hire ).
- Current Basic Cardiac Life Support (CPR) and Emergency First Aid Certifications (must be obtained within six months of hire and kept current thereafter).
- HIPAA Certification within 30 days of employment. Bloodborne Pathogens Certification within 30 days of employment.
PHYSICAL REQUIREMENTS ( The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.):
- Office work is performed indoors in a climate-controlled office environment and outdoors in a transportation vehicle where climate is controlled by driver.
- Must be able to lift or move at least 50 pounds or more.
- The employee is required to walk and sit occasionally/frequently.
- Must be able to sit for long distances and extended periods of time. Must have specific vision abilities including the ability to adjust focus.
