Discharge Planner

We at Norridge Gardens are seeking to hire an experienced Discharge Planner to join our team.

 Discharge Planner Expectations:

• Educate residents and their families about the discharge process, home care instructions, and available community resources.
• Serve as the main point of contact for family members or caregivers, addressing any questions or concerns during the discharge process.
• Maintain thorough and accurate documentation of all discharge planning activities and care plans.
• Ensure compliance with federal, state, and facility regulations and guidelines regarding discharge planning.
• Coordinate with insurance providers or financial services to ensure coverage and payment for post-discharge care, if applicable.

Requirements:

• Healthcare background (preferred)
• Must be a self-starter able to work independently and efficiently
• Reliable transportation
