Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a vital role in supporting individuals and families by providing comprehensive case management services. This position requires a strong understanding of behavioral health, and crisis intervention strategies. The Case Manager will work collaboratively with individuals to develop personalized care plans, ensuring they receive the necessary resources and support to achieve their goals.
Responsibilities
- Conduct thorough assessments of individuals' needs and develop individualized service plans.
- Provide crisis intervention and management to individuals in distress.
- Utilize motivational interviewing techniques to empower individuals in their decision-making processes.
- Mentor individuals and provide guidance on navigating social services and community resources.
- Supervise and coordinate care with multidisciplinary teams, including social workers, healthcare providers, and community organizations.
- Maintain accurate records of individuals interactions, progress notes, and case documentation in compliance with agency policies.
- Advocate for individuals’ rights and access to necessary services, including behavioral health support.
- Stay informed about relevant policies, regulations (such as Medicare), and best practices in case management.
Qualifications
- Bachelor’s degree in Social Work, Psychology, or a related field; Master’s degree preferred.
- Experience in crisis management, behavioral health, or child welfare is highly desirable.
- Strong mentoring skills with the ability to motivate and inspire individuals.
- Familiarity with social work practices and community resources available for families and individuals.
- Excellent communication skills, both verbal and written.
- Ability to work independently as well as part of a team in a fast-paced environment.
- Proficient in using case management software and other relevant technology tools.
- Commitment to maintaining confidentiality and ethical standards in all interactions.
