What are the responsibilities and job description for the Community Treatment Coordinator position at North Central Health Care?
**About the Role:**
As a Community Treatment Coordinator, you will be responsible for coordinating care for individuals enrolled in Comprehensive Community Services, Community Support Program, Children's Long-Term Support, or the Coordinated Services Team program. You will work closely with a multi-disciplinary team to provide community-based case management services.
Key Responsibilities:
- Determine ongoing eligibility for Community Treatment programs via the approved state functional screen.
- Facilitate the assessment and service planning process, including the identification of consumer needs, goals, objectives, strengths, barriers, interventions, and their priority in the treatment planning process.
- Assess ongoing needs, including crisis situations, to determine treatment interventions and service provision.
- Ensure the service plan and service delivery for each client is integrated, coordinated, and monitored.
- Provide services in adherence to the pillars of Community Treatment program philosophy, which include being strength-based, recovery-oriented, person-centered, flexible, trauma-informed, and consistent with best practice.
- Complete documentation in compliance with Community Treatment standards, agency policy, and applicable regulations.
