The Opportunity :
The Social Worker (BSW) is a strategic member of the treatment team and supports the patient with recommendations and referrals to other social services, placements in outside care systems, and coordination of care with family members and other providers regarding the needs of the patient.
This is a full-time, on-site position. The pay range for this role is $26.00 - $28.85, based on experience.

What You’ll Do :
- Responsible for timely completion of psychosocial assessments, Treatment Plan and Treatment Plan Reviews, and Discharge Summaries.
- Responsible for appropriate Case Management / Discharge Planning based on patient need and assisting in complex placement issues.
- Makes appropriate and timely referrals to other agencies based on patient need and services required.
- Performs social counseling services for individuals, families, and groups.
- Makes treatment recommendations to county corporation counsel and Human Service Departments.
- Contributes, participates, and supports the Interdisciplinary Team (IDT) approach to providing quality care for clients.
- Documents ongoing assessments and updates plan of care to reflect changes in client’s condition.
- Counsels with clients, families, or health care agents of clients regarding the plan of care.
- Serves as a client advocate.
- Attends and participates in assigned treatment staffing and other assigned committees.
- Responds to community requests for information regarding available services.
- Assures that patient care provided in the department is completed in a respectful manner that upholds the dignity and rights for adolescent and adult patients.
- Maintains regular on-site attendance
