What are the responsibilities and job description for the Jail/Re-Entry Navigator position at North Central Missouri Mental Health Center?
A. DEFINITION
The Jail/Re-entry Navigator role is embedded within the agency and local jails. This position offers support in the jail setting for individuals with behavioral health needs or presenting in a behavioral health crisis. The Jail/Re-entry Navigator is responsible for coordination among the jail, court, behavioral health provider teams, and community resources to ensure appropriate referrals are completed and all opportunities for diversion are explored. The Jail/Re-entry Navigator provides coordination upon release with all teams involved to ensure appropriate connections are made and the needed level of care is implemented. The Jail/Re-entry Navigator is a part of a person-centered integration team and works with local systems to coordinate care for individuals experiencing behavioral health crisis.
B. DUTIES
1. Participate in Crisis Intervention Teams (CIT) or other initiatives that provide opportunities for needed collaboration between entities working with individuals with behavioral health needs.
2. Collaborate with local behavioral health providers and resources in the community.
3. Provide consultation on behavioral health issues for jail personnel.
4. Assess individuals with behavioral health needs and support with de-escalation as appropriate.
5. Complete coordination of care with all appropriate parties upon release from jail, including jail, court, behavioral health provider, and community resource personnel.
6. Support and facilitate support for individuals with behavioral health needs with accessing needed resources and services.
7. Transport clients/individuals as needed.
8. Aid individuals with behavioral health needs with completing needed paperwork to explore diversion opportunities, community resources, and behavioral health services both during jail stay as well as upon release.
9. Assist jails with navigating inpatient hospitalization process when appropriate for those individuals who present a likelihood of serious harm due to mental health disorder.
10. Complete all mandatory documentation, as noted by supervisor.
11. Act as a member of the multi-disciplinary team.
12. Collect data and report supervisor and state.
13. Complete required admission (POE), intakes, and assessment to get the person established in services.
14. Complete jail outreach, including routine jail visits for those who are high utilizers of services.
15. Maintain and initiate clear, concise communication that is timely and accurate with all referral sources involved in the service provision.
16. Help the person increase his/her support network outside of the behavioral health service delivery system.
17. Participate in on call crisis rotation.
18. Review medications based on jail ability and cost effectiveness.
19. Coordinate with inmates’ lawyers on a case-by-case basis regarding treatment/services.
20. Assess inmates on suicide watch, as needed by the jail, to “take off” suicide watch.
21. Create resource packets that are available to inmates upon release from custody. Review every 6 months for accuracy.
22. Assist inmates with application for Medicaid so they can continue with services/medications upon release.
23. Assist inmates with coordinated medications to appropriate pharmacy for when they are released.
24. Other duties as assigned by the supervisor.
25. Maintain client confidentiality.
26. Perform related work as requested to carry out the purposes, goals and objectives of the agency.
C. ADDITIONAL REQUIREMENTS
1. Maintain current CPR certification and First Aid certification (if applicable).
2. Knowledge of trauma-informed principles to support individuals recovering from trauma, violence, and/or adverse life experiences.
3. Knowledgeable about behavioral health disorders.
4. Already have or ability to get Chauffeur’s License.
D. QUALIFICATIONS
Individual with a bachelor’s degree in a human services field including but not limited to social work, psychology, nursing, education, criminal justice, recreational therapy, human development and family sciences, counseling, child development, gerontology, sociology, human services, behavioral science, and rehabilitation counseling.
