Care Coordination Supervisor, Addiction Response Team(ART)
Position Title : Care Coordination Supervisor, Addiction Response Team
Position Status : Full Time, 40 Hours per Week
FLSA Status : Salaried / Exempt
Position Summary :
The ART Care Coordination Supervisor plays a critical leadership role, overseeing the day-to-day activities of Care Coordination CO-SLAW Addiction Recovery Team. This position is a first line leadership role and reports to the Program Manager of the Addiction Response Team. This role will primarily focus on providing the necessary support to grow, develop, and sustain a team of addiction professionals. It will involve collaborating with local providers and community agencies to ensure individuals can access substance use treatment and recovery services, while connecting them with resources that support their long-term recovery goals. Additionally, this role will support, expand, and oversee the delivery of recovery services throughout Northern Colorado.
Job Responsibilities
Support, Development / Mentorship and Programmatic Needs of the Care Coordination team :
o Support the Care Coordinators in understanding and performing primary activities including but not limited to using appropriate assessment tools, performing outreach and engagement of community members, community member and organizational education, building connection with new people to engage in treatment and recovery services, and organization and facilitation of community support groups
o Support persons enrolled in MAT to continue engagement with both medical and behavioral health treatment plans as needed
o Manage oversight of the completion and troubleshooting of GPRAs and other assessments with Care Coordination Staff
o Enter all documentation, contacts, and assessments into required databases
o Serve as a bridging structure between partner organizations and develop universal processes
o Daily oversight of Care Coordination Activities including NCHA & community engagement site representation, documentation, reporting, case reviews, and assignment of workflows
o Timecard Completion, shift scheduling, employee onboarding, staff education, performance reviews, professional development, and talent management
o Participate in case reviews with multidisciplinary team
o Support persons enrolled in MAT to continue engagement with both medical and behavioral health treatment plans
o Support Care Coordination staff with professional development and supervision
o Monitor members according to guidelines set by NCHA Policy and Procedure under the Addiction Response Team guidelines
o Understand, communicate, and facilitate on behalf of patients with regard to complaints, grievances, and appeal processes
o Regular case review with Care Coordination team and with the Program Manager
o Answer CO-SLAW hotline referrals and process in Healthcloud as needed to be directed to staff for outreach
o Problem solve unique situations with staff, and coordinate with other leadership as needed
o Strict confidentiality of member records and communications
Support the needs of the organization and Program Manager of Addiction Response Team and Senior Leadership of NCHA
o Coordinate with the Director of Addiction Response to ensure presence in NCHA offices, community partner meetings and programming
o Understanding and compliance of safety measures and protocols of the Addiction Response Team
o Understanding of and support with reporting and program deliverables including benchmarks, metrics, and KPI
o Support the Program Manager of Addiction Response in the pursuit of new funding to support the sustainability and growth of program offerings
o Coordination with Program Manager of Addiction Response,
o Assist Program Manager and senior leadership in ensuring all programs function within the approved budget and grant contracts
o Strict confidentiality of records and communications.
Support and foster the needs of the Community
o Develop, market, and facilitate community peer recovery professional network & coaching meetings.
o Coordinate with the Program Manager and senior leadership, the development of new program offerings for the community
o Cultivate and expand relationships with community organizations and potential partners
o Build and manage regional coalitions and community partnerships
o Participate in Housing 1st, Harm Reduction, Person-Centered and trauma-informed care trainings and practice
o Develop Care Coordination & Peer Support Agreements and workflows with partner organizations
Knowledge / Skills / Abilities

- Adjust priorities quickly as circumstances and needs dictate
- Perform with frequent interruptions and / or distractions from walk-ins, employee needs, leadership requests, etc.
- Computer skills including the navigation of multiple Electronic Medical Record systems, state administered data collection systems, proficiency in Microsoft Office products, and other programs as needed or requested
- The ability to coach employees on these systems as needed
- Partnership with local and regional health and human service resources
- Ability to break down silos and build trust between teams, organizations, stakeholder groups, and community
- Ability to build relationships and connect with historically marginalized populations including but not limited to people who use drugs, People of Color, People with Disabilities, LGBTQIA People, those experiencing homelessness, complex medical and / or mental health issues, and those who may be justice involved
- Experience with Public Health Data
- Deep understanding of the operations of the CO-SLAW Care Coordination Team
- Understanding of supportive professional development opportunities, particularly credentialing, of the Care Coordination and Addiction Workforce
- Strict Confidentiality of staff and member records and communications.
- Bilingual preferred (not required)
- Reliable transportation and a valid driver license and insurance
