Care Coordinator
Care Coordinators are part of the Addiction Response Team and are responsible for working in collaboration with local providers and community agencies to ensure individuals with substance use are successfully connected with resources to aid in their long-term health, treatment and recovery goals. Care Coordinators will aid individuals in meeting their goals toward ongoing recovery through education, outreach, connectivity to Medication Assisted Treatment (MAT) and other treatment and recovery services. Care Coordinators will report to Care Coordination Supervisor.
Role Summary :
This position works directly with the Poudre Library District & Community Behavioral Health Treatment Providers and systems including criminal justice, behavioral health, healthcare and community based organizations. The individual works to engage with library customers seeking assistance and resources with mental health, substance treatment and recovery, housing stability, food security and other security and safety needs, legal resources, and other needs that extend beyond the scope of customary public library reference and referral services. Role is split 50% of time onsite at the Poudre Library District locations and 50% in other systems locations.
Knowledge / Skills / Abilities
- Experience working and ability to build trusted relationships with persons with Substance Use Disorder, especially opioid use disorder, alcohol use disorder, stimulant use disorder
- Cultural Humility - Ability to effectively engage with different populations including People of Color, LGBTQIA , Disability, Veteran, Low / No Income and / or experiencing homelessness, People with criminal justice involvement, and people who use drugs, people with mental health conditions, people with complex medical conditions and more.
- Critical Thinking and Problem-Solving Skills.
- Customer Service Skills.
- Strong decision-making skills.
- Strong verbal and written communication skills (phone, email, and in-person).
- Ability to maintain non-reactive and calm when engaging with individuals who may or may not be intoxicated, belligerent, or have a history of behavioral health issues.
- Ability to adjust priorities quickly as circumstances and needs dictate for member success.
- Computer skills including the navigation of multiple Electronic Medical Record systems and data collection tools as determined by organizational needs.
- Experience and / or ability to partner with professionals in local and regional health, human service resources and criminal justice organizations.
- Experience working with Medicaid and Medicare.
- Bilingual preferred.
- Ability to work independently and in a busy office environment that may contain interruptions to work due to walk-ins, member needs, supervisor requests, phone conversations, etc.
