What are the responsibilities and job description for the Behavioral Health Therapist position at North East Medical Services?
The Behavioral Health Therapist (LCSW / LMFT) will provide mental health assessment, diagnosis, brief treatment, crisis intervention services and Warm Hand Off services for adult or adolescent members who present themselves with mild to moderate behavioral health needs. Practice within the Collaborative Care Model, working closely with multidisciplinary team members to achieve optimal treatment outcomes for patients. Perform case management duties and contribute to grant projects as required. Perform case management duties as required.
ESSENTIAL JOB FUNCTIONS :
- Provide culturally sensitive, linguistic appropriate assessment. Follow NASW code of ethics.
- Conduct a thorough bio-psychosocial assessment interview with patient
- Develop comprehensive treatment plans and provide evidence-based therapies, including Motivational Interviewing, CBT, TF-CBT, DBT, ACT, EMDR, and more. Maintain ongoing engagement with patients to support the improvement of their behavioral health conditions. Collaborate with PCP, psychiatrist / NP, to supports and counsels patients with psychoeducation, compliance with prescribed medication regimens. Communicates concerns about medications or side effects to PCP / / psychiatrist.NP
- Maintain a regular case load. Provide Warm Handoffs between primary care providers and NEMS patients.
- Documents treatments and progress in accordance with state regulations and department procedures. Prepares reports, intakes, treatment plans, and case summaries and maintains ongoing confidential records.
- Provides case management, referrals to other services (e.g. county ACCESS line, substance abuse treatment, specialty care, and community resources) as needed.
- Submits timely billing record to the billing department to enable billing for behavioral health services.
- Under supervision of the Behavioral Health Manager, develops and implements both the quality improvement and outcome evaluation programs for behavioral health services.
- Participate in grant project, will co-facilitate department case consultation meeting.
- Performs other job duties as required by manager.
