The Member Services Social Worker I carries out eligibility screening for clients and prospective clients on a regular basis; conducts psychosocial assessments, assesses the health and related needs of prospective clients and makes appropriate referrals; advises and counsels clients regarding a variety of personal matters.
ESSENTIAL JOB FUNCTIONS:
- Conducts psychosocial assessments for perinatal patients.
- Participates in Comprehensive Perinatal Service Program Committee.
- Advises and counsels clients regarding family, economic, physical, and emotional matters. Provides assistance or referral in resolving these matters whenever appropriate.
- Informs prospective clients of the services available through NEMS.
- Conducts interviews to identify and determine patient eligibility for NEMS sliding fee program, public health and social; registers clients for NEMS service; obtains demographic information; assists in completion of applications and forms; and verifies insurance eligibility.
- Completes assessment on health and social needs and makes referrals to other NEMS services and community health agencies as appropriate.
- Completes all training programs and certifications; maintains an active Covered CA Certified Enrollment Counselor certification as required. Participates in training and meetings as appropriate.
- Provides one-on-one or group education on health programs, social programs, and important changes related to these programs as needed.
- Provides confidential and reliable enrollment services to individuals and families. Enrollment activities include application assistance for Medi-Cal, Covered CA, Healthy Kids (HK), Healthy San Francisco (HSF), Medi-Cal (MC), CalFresh, and other public cash aid programs as they become available.
- Responsible for case management activities consisting of one-on-one follow-up, tracking, reporting with the ultimate goal of connecting individuals and families to health coverage programs.
- Investigates and resolves client problems involving enrollment issues with Medi-Cal Covered CA insurance, and other health coverage programs.
- Maintains needed data on clients; does recording in clients’ medical records; and prepares statistical reports as required.
- Maintains up to date on eligibility and changes of all public programs.
- Perform Member Services duties on an as needed basis
- Performs other job duties as required by manager/supervisor.
QUALIFICATIONS:
- Bachelor’s Degree in Social Work. Two years of work experience in social work or related field preferred.
- Knowledge of publicly funded health coverage, social programs, and cash assistance program such as CalFresh, GA, SSI, Healthy San Francisco, Medi-Cal, Covered California, Healthy Families, and Healthy Kids, as well as the Affordable Care Act (ACA).
- Maturity and understanding of human behavior as related to interpersonal relationships.
- Excellent interpersonal, written, and verbal communication skills. Must be detail oriented with excellent time management skills.
- Must be proficient in computers.
- Ability to multi-task and work independently.
- Ability to interview people of all ages and socio-economic level effectively.
- Knowledge of state, city, and community social services agencies.
- Prior experience and/or training in medical social work preferred.

- Constant ten key typing
- Constant fingering (fine dexterity) and handling (grasping, holding)
- Constant repetitive motion of hands and feet
- Constant sitting/standing/walking
- Occasional reaching over shoulders and over head level
- Rarely lifting/pushing/pulling objects over 10lbs
LANGUAGE:
- Must be able to fluently speak, read and write English
- Must be verbally fluent in Chinese (Cantonese or Mandarin)
- The ability to speak other Chinese dialects is an asset
STATUS:
- This is a FLSA Non-exempt position.
