SUMMARY OF POSITION:
Under the direct supervision of the PACE Center Manager, the PACE Home Care Supervisor is responsible for supervising a team of clinical and non-clinical staff who provide home-based or center-based personal care, center services and housekeeping support to PACE Participants to support their continued ability to live safely and independently in the community. The Home Care Supervisor works closely with the PACE Community Services RN on meeting Participants’ home care needs.
ESSENTIAL JOB FUNCTIONS:
- May serve as a member of the Interdisciplinary Team (IDT).
- Arranges for, oversees delivery, and provides on-going management of:
- Home care services as per Participants’ established plan of care.
- Coordination of center services
- Any supplies needed at home, as per the care plan (e.g., incontinent supplies).
- All Durable Medical Equipment that is utilized in Participant’s homes.
- Supervises the performance of caregivers to ensure that they provide high-quality service and care to the Participants.
- Assists with identifying qualifications, competency, and performance expectations for home care staff, in consultation with the PACE Center Manager, PACE Community Services RN, and NEMS Human Resources.
- Oversees training and competency checks for caregivers.
- Coordinates care needs with Participants and Participants’ family caregivers.
- Reviews reports from caregivers and communicates Participant changes to IDT members in a timely manner. Ensures that all Participant changes are documented appropriately in the medical record.
- Remains up to-date on home care standards and best practices.
- Provides effective leadership by coordinating and integrating services within the department, with other departments and with the IDT’s primary functions.
- Attends IDT meetings, as required or necessary.
- Provides timely and necessary communications to Participants to convey staff or in-home care plan changes.
- Oversees the day-to-day coordination and management of services provided by caregivers.
- Supports enrollment of prospective Participants in the PACE program.
- Utilizes Epic appropriately and efficiently and serve as content and workflow expert for resolving issues and proposing solutions in Epic
- Performs other job duties as required by Manager/Supervisor.
QUALIFICATIONS:
Required:
- Associate’s degree or higher in nursing or related field.
- Current BLS and First Aid required
Preferred:
- At least one year of experience supervising staff
- Current unrestricted California licensure/certification as a Licensed Vocational Nurse or Certified Nursing Assistant.
- Experience in caregiving, home health, or other community-based care
- At least one year of experience working with the frail and/or older adult population, or willingness to receive appropriate training upon hire.
LANGUAGE:
- Must be able to fluently speak, read and write English.
- Fluency in Chinese (Cantonese and/or Mandarin) is required.
- Fluency in other languages is an asset.
STATUS:
This is an FLSA exempt position.
